    Add up to 10 different places that were significant in your loved one’s life. You may also include dates and a short description of why each place matters. These descriptions will also be used to create an optional spoken narration for the Map of Life. Example: Edinburgh, 1st February 1945 – John was born London, 1968 – Graduated from university Paris, 1972 – Honeymoon with Mary You can also let us know your preference for the narration voice — for example, male or female, and if a particular accent or tone would feel most meaningful. 💡 Tip: If it’s easier, you can write the biography in a document first, then copy and paste it into this box. Think about places tied to milestones: birth, marriage, achievements, favourite holidays, or family homes.

  • Dedicate a Star *

    Please provide the name you would like for the star. You can also explain why you chose this name, for example linking it to a special memory, date, or place. Example: “Star of Edinburgh” – because John was born there “Evening Rose” – to reflect her love of gardening 💡 Tip: If it’s easier, you can write the biography in a document first, then copy and paste it into this box. You can be creative — many families choose names linked to love, places, or shared experiences.

  • Ceremony / Grave Location *

    Add the location of the burial or cremation, or where the ceremony took place. You may include a cemetery name, town, or GPS link.

  • Charity Information *

    If donations are being collected in their memory, you can include the name of the chosen charity and a link (e.g. “Alzheimer’s Society – https://alzheimers.org.uk”).

  • Hosting Duration *

    Your QRest™ memorial page will stay online for the duration you select. You can extend hosting later if needed. Pricing is based on the chosen duration.

  • Delivery Time *

    Standard delivery time is 5 working days. If you need the page completed urgently, select Priority for delivery within 48 hours (Monday–Friday).

  • Would you like to protect this memorial with a password? *

    Please choose a password for your memorial. Be aware that passwords are case-sensitive (for example, "Memory123" is different from "memory123"). The password can be changed at any time by request via email, and you will also receive a password reminder for your records.
