  • Name *

    The full name of your loved one to be remembered.

  • Tagline *

    A short phrase or quote that captures their spirit.

  • Date(s) *

    Important dates to honour, such birth and passing.

  • Biography *

    Share a written tribute (up to 1000 words). This could include life milestones, character, achievements, or anything meaningful you'd like to include.

  • Upload Photos *

    Upload up to 5 photos that best represent your loved one. These may include portraits, family memories, or significant places.Max file size: 1 MBPermitted file types: jpg jpeg jpe gif png

  • Timeline *

    List any important dates and life events (e.g. “1965 – Born in Edinburgh”, “2002 – Retired from teaching”). This helps visitors see their life journey at a glance.

  • Ceremony / Grave Location *

    Add the location of the burial or cremation, or where the ceremony took place. You may include a cemetery name, town, or GPS link.

  • Charity Information *

    If donations are being collected in their memory, you can include the name of the chosen charity and a link (e.g. “Alzheimer’s Society – https://alzheimers.org.uk”).

  • Hosting Duration *

    Your StoneCode™ memorial page will stay online for the duration you select. You can extend hosting later if needed. Pricing is based on the chosen duration.

  • QR Plaque / NFC Tag [copy] *

    Choose between a QR code plaque, an NFC tag, or both. QR plaque: A weather-resistant plate with a scannable code NFC tag: Tap your phone (no app needed) to open the memorial

  • Optional Name and Date for QR Plague *

    You can choose to add a name, or / and date (s), above / below the QR code on the plaque — but not on the 4×4 cm size. Please add it here. If left blank, I will send only a QR code.
